It's a while since this enquiry but one idea is RF getting into the PC down the USB cable and resetting the USB port. I've had this myself.
The usual things that reduce RF should help, Common mode chokes on the antenna feeder, keeping the feeder away from the USB cable, good quality feeder.

What I found was that if the LoTW entries were new ones it was fine with any number of entries.
BUT if the entries were already in LoTW then trying to add them again caused the failure.

This problem with Log4OM hanging after a TQSL upload is started has been around for some time and seems to affect a significant number of users. I thought I'd try running tqsl with the parameters from the log file in a command prompt:

Thanks, tried that. no difference. Log4OM still hangs and needs closing using the Task Manager.
Discovered that the TRACE setting has to be enabled every time but have log data for the close. The last LOTWManagement lines are:

I've tried with trace enabled and obtained a log with a lot more information.
From what I can see TQSL.exe is started but never returns anything. Log4OM sits waiting and that's it.
Can I post the log somewhere? I don't seem to be able to send PMs.
