Problem uploading to LOTW
In QSL Manager I select required QSOs and they are displayed correctly. The sent drop box is at "Requested". After Clicking upload selected QSO and then the green upload arrow I will see either "managed mode set" or "LOTW END". Nothing is uploading. Any ideas what might be happening?

Let me second this complaint. I am using 2.28.0.0. I usually upload and download to LoTW twice a week or so. On October 7, I used the usual procedure and it worked. Around October 15, I did it again and it looked like it worked. Yesterday I tried again and while it looked like the upload was good, and all of the QSOs are marked "sent" in Log4OM QSL Manager, I notice that nothing since October 7 is actually on LoTW. Yesterday I also installed the requested updated to TQSL, so that is current. Just now I uploaded two new QSOs, sort of a test, and it seemed to work, but then LoTW takes time to show things so I can't tell if it really worked.
The QSL manager shows 0 downloads, but that is logical because LoTW itself, on its web interface, shows no new QSOs. So has LoTW been down for two weeks (which I think I would have heard about on QRZ) or is there an issue with Log4OM?

Using version 2.29.0.0 and latest TQsl update I just uploaded fine as the log file below:

So it's working for you. But then why is it not working for me, when I'm not even seeing an error in LoTW? I may have to go back to manually uploading my ADIFs.

Since you noted using V 2.28.0.0 ... did you delete and rebuild your UDP Inbound Connections as noted in the Release Notes for V 2.27.0.0 ??
There are options inside those rebuilt UDP Inbound Connections that will advise Log4OM what to do with respect to the external services uploads for the QSOs brought in from the FTxx APPs. I've added an image of those options inside the "rebuilt" UDP Inbound Connections ... depending upon which elements you may select.
Reading this post and the one, where you replied to Terry, I'm guessing that you use the QSL Manager to 'manually' upload and download your QSOs to LOTW. This as opposed to the 'automatic upload on close of Log4OM' ???

What version of Log4OM are you using?
If it is V 2.27.0.0 or newer (current release is: V 2.29.0.0) you will need to delete and rebuild your UDP Inbound Connections and make choices inside those connections to address QSO uploads, etc.. See image I included in post above to Fred.
FWIW I don't use the "Requested" option for dealing with QSO uploads, etc.. I keep it simple (for me anyway) and just use "Yes" or "No". If you like you can select such in the Configuration section for Confirmations and External Services / LOTW. Should you make such a change you can then also tweak the Requested label to Yes or No in the QSL Manager panel.
Also ... is this impacting the QSOs you are getting via an FTxx APP through the UDP Inbound Connection port? If so you may not have deleted and rebuilt the UDP Inbound Connections ... depending upon which version of Log4OM you are using. See 2nd graf above.
I've never seen an alert/message with "managed mode set" so I can't specifically address that. Terry might have seen such and if so he might add input when he can.
Your question has prompted many questions from me to help peel the onion to get your items resolved.

I have no problem with inbound -- I've been using 2.28 for a while and Log4OM does receive all of my contacts. The log is complete and the QSL Manager part shows that the contacts have all been uploaded to LoTW. But LoTW has shown no new QSOs since Oct. 7, and new confirmations aren't being uploaded either.
But I do note that on web LoTW, it shows no new QSLs since Oct. 7 either. LoTW does show the two QSOs I uploaded (as a test) this morning, but nothing in between today and Oct. 7. Log4OM says they're uploaded but LoTW shows no TQSL activity between today and Oct. 7, even though there were two uploads.
I'd try reuploading them, but since they're marked "Yes" in the LoTW Sent column, they won't, unless I can bulk-edit them to "No" in that column. How do I edit contacts in bulk?

You use the QSL Manager (LOTW dropdown) to edit the Yes/No, etc.. Tic the "Enable update" checkbox and the edit panel will appear.
Select one or two Q's first to edit / test before you do a whole mess of them. Just get comfortable/familiar with the Edit option and you will be good to go.
FYI - I upload (via QSL Manager) to LOTW 3-4 times each week and also ask for LOTW downloads probably 6+ times each week. I've not noted any upload/download issues as you have experienced.
Also all my Q's are strictly CW so all manual or cluster entry into my Main UI. I choose to 'not use' the LOTW Auto Upload on Log4OM close. I just prefer to keep my fingers on what I'm doing as best I can.
I do occasional testing of FTxx modes, but have also not experienced any upload/download issues there either.

OK, thanks. That button made the difference, opening up a box that let me mark a range of QSOs as not yet uploaded. AFTER doing that, I asked again to upload, and got the usual positive response from TQSL. And THEN I asked for a range of dates to be downloaded, and QSOs from Oct. 8 onward suddenly showed up! Logging in to LoTW directly again, the recent QSOs window now shows those contacts, as well as a whole bunch of QSLs received since Oct. 7, which were not there earlier this morning.
Very weird! It actually looks as if re-doing the upload on Log4OM un-jammed something in LoTW that went awry on a previous attempted upload.

I follow the ARRL-LOTW Reflector and have seen some minor hiccups with TQSL, but (as best I can discern) they were related to modifying TQSL to still support OMs who are still using Windows XP!!!

The most recent update to TQSL fixed some issues Rick had noted when he was doing some update programming.
Dunno if that would have been the cause of such an issue and I have no way of trying to get to the bottom of such either.
Regardless ... so glad that whatever was the issue has decided to better cooperate with you. Kudos !!
