I'm a bit OCD and not knowing why will drive me insane.
So, I'll do the re-install later or tomorrow a.m.
If it fixes it I will go crazy; one checkbox in dozens not working.
BTW, I performed a full windows install immediately before downloading the latest Log4OM.
I will try the re-install, I'm just saying...

After this is put to bed, I used to have ccleaner installed, it was recommended for a specific problem. I didn't reinstall it after the windows reinstall.
for future reference I'd.love to know what its problem is.

Right, uninstall, rebooted laptop, reinstalled it. Fault remains.

Something to consider...
I have installed this version onto a blank machine, I.e., I have not upgraded to this version. There are no legacy objects or settings.

Check firewalls or anti virus software is not blocking Log4OM

... from setting a checkbox, really?
If one was blocking then it wouldn't upload to QRZ after I had manually entered a QSO but it does. Its ONLY the checkbox that's defective, NOT the upload function.
There's no flag set in the database to enable it to automatically upload a QSO copied using the ADIF Monitor.
So, the upload feature works just not automatically.
I'm struggling to make it any clearer.

As I said earlier I have tested using the same Log4OM release that you have and it works fine, the setting is persistant after several restarts as shown below

There have not been any other reports about this from any of our 40K users
So it has to be a local issue on your maching, that can only be a bad download or install or something interfering with the save and apply routine.

I have no idea how many clean installs have been made but there has been a large number of downloads
