When Log4OM2 connects to the radio it keys the transmitter and keeps it keyed. The frequency readings of the radio are displayed.
I have verified my CI-V settings in the radio are the same as other users.

First off a couple questions:
What version of Log4OM are you using?
What CAT APP are you using? Hamlib, Omnirig, etc., etc..
Which version of either Hamlib or Omnirig, etc.?

I don't have an Icom so am going by support given to other OMs here on the Forum.
Few things to look at:
Start with Page 26 of the "current User Guide". An IC7300 is noted, but some of the items apply across many of the IC rigs. There are steps and settings there for both the IC and Log4OM.
There are currently 3 "ini" files available in Omnirig. Have you tried the other two that are available? I have a screen clip of the three that are noted in my Omnirig "rig" folder.
The notes on Page 26 direct you to various tic boxes in Configuration / CAT Interface that may also impact the situation.
Also ... I strongly suggest that you update Log4OM to the current version that is: V 2.25.0.0 ... There have been many changes since 2.19 ... I would update that first and give it a go as you currently have things arranged and then start looking into other Omnirig IC ini options, etc..
