This issue is finally resolved. For some undetermined reason, the LotW Sent field wasn't updating to "Yes" after a manual upload using QSL Manager. This happened after a single QSO was logged several months ago and all subsequent QSOs didn't update to "Yes". At Terry's direction, using QSL Manager, I bulk edited all of those fields to "Yes" and tried again with new QSOs. It worked after that, QSOs update to LotW sent "Yes" after that.
My guess is that there was an invisible control character or some other anomaly in the first line of data that kept subsequent lines from uploading. Editing the field apparently removed that.
