When I queue up QSL's for printing the address shown is never complete or in the right format and I have to go to QRZ, search for the call, copy and then past the correct address into the QSL card template before printing. Is there a fix for that?

All that you need to do is click the green buttons indicated in the screenshot below to update the address of the station or his QSL manager - No need to cut and paste.
See the user guide for full instructions
You do need to either have your options set to QRZ if you are a paid subscriber or to the free HamQTH for this to work

Yes. Well in my case, when I update I get the name in the proper field, then in the address field I get:
City
State
Country
In that format. No address, No zip code, and as in the example above not in the proper format
Address
City, State, Zip Code
I have checked QRZ as well as HamQTH and both are correct and I'm able to log into each of them, and they are set up properly.

I did not ask, but I assume that you have completed the Settings 1 menu and in particular the area within the red box in the screenshot below?

Make a backup of you data
Uninstall Log4om
Download a fresh 1.15 Log4om
Install the new download.
The only thing I can think of apart from you having used the wrong user name and/or password for QRZ/HamQTH is that you might possibly have had a bad download or install.
What version of windows are you using?
