Where did you save te club list to?
-> file on harddisk

As you can see from my screenshots, the clubs shown in the extended window are different from the ones selected in the settings window

Did you add the club to the settings/resources editor/clublist.txt file? and save it
Email me your csv file of members that you used

I've attached the FOC club members file in this post.
I now have stored it in the C:\.....\appdata\roaming\Log4OM2 as you've suggested.
Did you update the clublist resources file?
-> I've now done that and can confirm that FOC now indeed shows up in the Extended Window, very good.
However, as soon as I QSO the next callsign, the "Contact Club & Assoc's" dropdown is wiped and thus no club info is shown.
The orange [FOC] in the QSO entry window shows for some members, but not for all.
For example, it shows for VE3EJ, but not for DJ6SI.

Your file is not going to work it has many duplicate membership numbers, each member must have a UNIQUE membership number
Also there are some lines with multiple call signs and superflous data
Log4OM expects and searches for the unique call sign entered and expects there to only be one membership number attached to that call. As an example there are eight call signs all with the number 1708

thanks for analyzing, well yes that is by design - as many members hold different callsigns and they are all mentioned in the file.
I'll try to make a file with only the primary callsigns.
